Affidavit of Discrepancies (English & Italian)

** NOTE: Not all consulates accept this form when there are discrepancies. They require that you get a revised copy of the Vital Record with the errors corrected.

This is a boilerplate Affidavit of Discrepancies. This can be included with your application if your relatives have different names on different documents.

For example, if your relative's name at birth in Italy was Anthony but all his American documents say Tony, you can include this document to swear that they are one-in-the same person.

Included with this purchase are the following two files:

1. A .docx file of the Affidavit of Discrepancies in English
2. A .docx file of the Affidavit of Discrepancies in Italian

These are editable documents files so that you can add as many discrepancy clarifications as needed.
